Care Managers & A Social Worker
Navigating the healthcare system can often be confusing and frustrating for patients, but you don’t have to go it alone. Did you know that Inter Valley Health Plan has an expert staff to help you get the most out of your health care? We have care managers (also known as case managers) and a social worker who provide support and a variety of solutions for our members. Every week they meet with an interdisci plinary group and other health professionals to present and discuss the care of their members. A psychologist and a chaplain also work with our physician medical director to oversee our efforts.

Care Manager Services
Every situation is different, but in general, our care managers can:
• Clear up any questions a medical provider has about our coverage.
• Contact a hospital to ensure timely care and reduce any potential delays in treatment.
• Provide helpful educational materials about various medical conditions.
• Develop a program to help you manage cancer, diabetes or congestive heart failure.
• Plan a strategy to keep you safe and healthy after leaving the hospital.
• Provide many other services and resources to help you stay healthy.


Social Worker Services
Our social worker assists people with a wide array of issues. For example, our social worker can:
• Help you apply for Medi-Cal.
• Locate community resources to help you, such as transportation services and grocery stores that make home deliveries.
• Assist you with the difficult process of deciding to move into a retirement home or assisted living facility, with an emphasis on helping you live at home as long as possible.
• Introduce you to government programs that pay for in-home care to assist you with day-to-day needs.
• Talk to you about issues and concerns involved with being a caregiver for a loved one.
• Assist with a variety of other social, environmental, and emotional issues.
