Care Management Program
Care programs are available for Inter Valley Health Plan members. Providers, hospital discharge planners, social workers, and other ancillary health care team members are encouraged to refer members to these programs when appropriate.
 

 Care Management Program for Seniors

  • Wellness Library. A comprehensive web based education and information for members. Includes a variety of topics such as Senior Health, Heart Health, Diabetes, Medication, Exercise and Fitness and much more.
  • Disease Management. A program designed to support members with chronic conditions like diabetes and congestive heart failure. The focus is on providing members with resources to restore health and maintain wellness.
  • Care Management. An element of the Disease Management Program allowing members to work with a nurse care manager to understand their disease and access needed care resources.

 

These programs are completely voluntary and offered at no additional cost to you. The care team consists of dedicated nurses and the medical director. They will work with your doctor to develop and coordinate a plan of care that meets your unique needs. A nurse is available by telephone 24 hours a day seven days a week. All medical information obtained during the care coordination is kept confidential and is only used to support your individual care plan.
